Binibining Pilipinas Universe 2004 Maricar Balagtas wore a national costume inspired by the Pintados of the Visayas region in the Philippines. During the 16th century, Spanish explorers came to the Visayas region and they encountered native men who were heavily tattooed; the tattoos were symbols of bravery and honor obtained during warfare. Because of their extensive tattoos, the Spanish explorers called the Visayan natives as "pintados" (the painted ones). Maricar's body suit depicted the intricate tribal tattoos while her skirt depicts tree bark and silk fibers. She also wore a hairstyle adorned with gold ribbons and flowers. To complete her look, she held large fans made of peacock feathers.

The back portion of her backdrop featured paintings of important historical events in the Philippines and a verse in the Philippine national anthem, written in ancient baybayin script.
Miss Universe Philippines 2019 Gazini Ganados wore a fantasy national costume that celebrates the Philippine eagle. The Philippine eagle has been the national bird of the Philippines since 1995 and laws were enacted for its protection and conservation. Depicting a goddess, Gazini's all-silver attire depicts life-sized eagles on both her shoulders. Her headdress depicts artistic eagle wings and she also reveals a pair of wings from her waist area. Gazini was later awarded as Best in National Costume.
Miss Universe Philippines 2020 Rabiya Occeña Mateo wore a national costume that celebrates the colors and symbols of the Philippine flag. Dressed in a glamorized leotard, Rabiya's costume depict large angel wings and a modified train. One side of her costume is colored blue which represent peace while the other side is colored red, which represents courage. Her costume also featured the three yellow stars which represent the three major regions of the Philippines - Luzon, Visayas and Mindanao.
Miss Universe Philippines 2021 Beatrice Luigi Gomez wore a fantasy national costume that depicts Bakunawa. In the Visayas region of the Philippines, Bakunawa was believed to be a mythical sea-serpent that would swallow the moon and was the cause of eclipses. Beatrice Luigi's costume depicts green scales with two sea-serpents in gold, swallowing the moon. Her ornate headpiece is an artistic depiction of the moon.
For her national costume, Miss Universe Philippines 2022 Celeste Rabimbi Coretesi depicted the local Filipino super hero, Darna. Created by Cavite-born Mars Ravalo in 1950, Darna started out in comic books and was soon depicted in local films and TV series, eventually becoming an icon in Philippine pop culture. Celeste depicted Darna's costume consisting of a bikini set, loincloth, boots, wrist guards and headpiece. To further enhance her Darna costume, Celeste carried a back piece depicting the eight-ray sun symbol from the Philippine flag.
Miss Universe Philippines 2023 Michelle Marquez Dee wore a national costume based on her background as a sergeant in the Philippine Air Force. Her glamorized pilot-inspired costume depicts traditional rattan weaving, which is common in the Philippines. Michelle carried a back piece that depict airplane wings that features the elements and colors of the Philippine flag. The back portion of her airplane wings features murals of tourist attractions and the country's tourism campaign. Michelle was later awarded Best in National Costume.
